THE PROBLEM
Follow-up is booked by the date, not by need
Between appointments, the service cannot see how each patient is doing. So the safest choice is to book everyone back in, and let the calendar decide who is seen next.

Each pathway asks for structured responses at every stage, so clinicians can see who is stable and who needs to come back in. They review the responses as structured forms in a worklist.
Royal Berkshire's rheumatology service, with DrDoctor as one of two technology providers, ran this from 2019 to 2024. Over that time, new appointments a year rose more than sevenfold, from 427 to 3,129.
